PRC Professional ID Card Renewal Requirements

Renewal Requirements of License ID for all Courses:

1. Duly accomplished renewal form/s
2. Two (2) pieces passport size (1 copy only for Teachers) RECENT Colored Picture in PLAIN Background with complete nametag
3. One Photocopy of your latest Professional ID Card
Note: In case you lost your Professional Identification Card, submit duly notarized Affidavit of Loss and photocopy of your Certificate of Registration (Lupon)
4. Marriage certificate from NSO (in Security Paper) for female married applicant who wishes to apply for change of name due to marriage
ADDITIONAL REQUIREMENT AS PRESCRIBED BY RESPECTIVE BOARDS
a. GEODETIC ENGRS. -G.E.P. MEMBERSHIP ID OR CERTIFICATION
b. MECHANICAL ENGRS./PLANT MECHANICS. - PSME MEMERSHIP ID OR CERTIFICATION
c. ELECTRONICS ENGRS. -IECEP ID OR CERTIFICATION
d. ARCHITECTS - UAP MEMBERSHIP ID OR CERTIFICATION
e. ELECTRICAL ENGRS./RME. - IIEE MEMBERSHIP ID OR CERTIFICATION

    i was robbed last saturday and my id was inside mywallet.. how can i get my new id.. howmuch do i needto pay?..

    some professionals have fake PRC IDs, How will I determine a fake from a real one?
    Why not impose death penalty to the ones using fake IDs.

    will PRC Davao honor a downloaded teachers' license renewal form?are they available for online license renewal for prof. teachers and pay corresponding fees through an authorized bank?
