1. Any of the following:
a. Report of ratings
b. Notice of admission
c. Official receipts
2. Current Community Tax Certificate (cedula)
3. Updated NBI Clearance for professions such as criminology, CPA, chemical engineering, MDO, MEO
4. Updated Medical Certificate for professions such as MDO and MEO.
5. Certificate of refresher in the following professions: (fish tech,agriculturist, CPA, Med Tech, Physical/Occupational Therapy, Medicine)
